To register for FC Samoa Academy:

1. Download and complete the YOUTH CLUB REGISTRATION CONFIRMATION  [ Word | PDF ]    from the FC Samoa web site.
2. Bring the registration form, a copy of your child’s birth certificate and $60 ($30 registration fee plus $30 monthly Academy fee) to the first session.
