HOW TO REGISTER

Step 1. Join FC Samoa (New and Returning Members only. Current Members can skip this step.)

• Mail: Copy of Birth Certificate to FCS Registrar
  (New Members only -- We keep these on file, so you only have to do this once a lifetime.)

• Mail: Annual Membership Form to FCS Registrar
  (This needs to be done once a year… expires July 31st)

• Send Payment: see options below.

Step 2. Signup for a Course

• Mail: Academy Course Signup Sheet to FCS Registrar
(coming soon -- registration opens mid-late October)

PAYMENT OPTIONS

Payment By mail:

$40 Annual Club Membership
Membership expires July 31
Make checks payable to “FC Samoa” include payment with forms
Please note on the check: Annual Dues - Player’s name

Class registration
Make a single payment of $95 for 3 months of Academy training -OR- $40 initial payment (plus two remaining $40 payments that are due by the 5th of each month)
Make checks payable to “FC Samoa” include payment with form(s)
